Health Care First Partnership has an opportunity for a qualified Advanced Clinical Practitioner to join our established team and to help us deliver excellent patient care and improve the health and well-being of the people within our local community, we serve the health care needs of around 33,000 patients.
We welcome applications from Nurse or Paramedic Practitioners with an MSc in Advanced Practice and who share our vision of providing the best possible general practice care to our patient population, of working in a caring team and of seeking innovative solutions to the challenges of the ever-changing NHS. Experience in managing chronic diseases (Asthma, COPD and Diabetes) within primary care would also be an advantage.
